1) I hereby declare that the registered participant is medically and physically fit to participate in West Life Saving Club Development Sessions and will declare any medical/health/fitness or other concerns to coaches prior to beginning the program or prior to joining any session should a problem present itself.
2) All participants & their families agree to show respect at all times.
3) I acknowledge that by registering in this event that I will become a Royal Life Saving Society Western Australia sports member and will be coverage by their insurance and will receive email notifications until I unsubscribe online.
4) I give consent for photograph/s taken of the registered participant during participation to be utilised by West Life Saving Club and Royal Life Saving Society Western Australia authority to be used in promotional and advertising material for West Lifesaving Club’s and Royal Life Saving’s purposes. These photographs will not be on sold to any third party and will only be used in the promotion and advertising of programs and initiatives.
5) I acknowledge that participation in this program is entirely at the registered participant’s risk.
7) I acknowledge that it is the responsibility of the participant to carry all necessary medications including allergy medication and that coaches will not administer medication.
8) I agree to dissolve West Life Saving Club and its volunteers & coaches of any liability that may arise from the registered participant’s participation in the program or activities.
9) I understand that there will be no refunds provided in the event that the registered participant is unable to attend a prepaid Development Session/s.
